Request for Accommodations
Who Can Request an Accommodation?
Students with a documented physical or mental disability may request a reasonable accommodation.
Accommodation requests may involve:
- Permanent disabilities or conditions
- Newly diagnosed disabilities or conditions
- Short-term conditions
- Physical disabilities or limitations
- Mental health or psychological conditions
- Illnesses or medical conditions that create functional limitations
BTC may also approve short-term accommodations for circumstances such as surgery, hospitalization, or treatment for illness.
When Should I Request an Accommodation?
Submit your request after admission into the college and before classes begin whenever possible.
Students with a known disability requiring accommodation should submit their request after admission but no later than the first day of class. Early notification allows the college adequate time to review documentation and coordinate approved accommodations before coursework begins.
If you are newly diagnosed or experience a change in your condition after classes have started, notify the administrative office as soon as possible so your request can be reviewed.
How to Request an Accommodation
1. Complete the Request Form
Students with a known disability requiring accommodation should submit a request after admission but no later than the first day of class. If a disability is newly diagnosed after classes begin, notify the administrative office as soon as the diagnosis is made.
2. Completed forms should be emailed to emock@bolivarcollege.edu or can be submitted to the front office.
You will be asked to describe:
- The disability, illness, or condition creating the need for accommodation
- The accommodation you are requesting
- How the requested accommodation will help you participate in your educational program
- Whether your condition is permanent, newly diagnosed, or short-term
